Just taking a first look at the process and downloaded the forms. We are a family of 3 with both myself and my husband working in Switzerland. Do we each complete a separate application form? ..and do we both include our daughter on the form? Or is it a family application? Grateful for any pointers. Thanks!

You need to fill one form per family.
"Attention : ne remplissez qu'un seul formulaire et qu'une seule déclaration de ressources par foyer.
Ces documents concernent également vos ayants droit, c'est-à-dire votre conjoint (ou votre concubin, ou votre partenaire lié par un pacte civil de solidarité (PACS), vos enfants (des conditions d'âge sont prévues), vos ascendants ou vos descendants (vivant sous le toit de l'assuré et se consacrant exclusivement aux travaux du ménage et à l'éducation des enfants à la charge de l'assuré), ou toute autre personne vivant avec l'assuré depuis plus d'un an et se trouvant à sa charge totale, permanente et effective.
Les ressources prises en compte correspondent au revenu fiscal de référence figurant sur votre avis d'imposition."
