For the "attestation de départ," You need to go to the office cantonal de la population on Route de Chancy 88. When you enter the building you will see a whole stack of forms. It's form D if you are leaving.(maybe you can find the form on the internet)
The AVS (pension) that your company paid whilst you were working here to a Swiss pension company will be paid back to you when you leave... you will get 100% of it back, which you can either use to invest in property, or reinvest ...
