My GF is in the process of asking for a b-permit and to submit the request she has to declare where she lives.
Of course she is at mine and on the form for the request the only option I see could be a match is for me to declare her as living in "sous-location". I called the regie and they say that a declaration de domicile should be good.
My questions is: what should I write in it? what kind of declaration is it? is there a specific form I should fill in?

There is no special format. You should confirm that your girlfriend resides with you and join a copy of your passport or ID card.
In French: "Dans le cadre d'une demande d'obtention d'un permis B, je confirme sur l'honneur que [NAME], née le [date of birth], réside avec moi au [address]". [Signature]
