Paperwork you have to file when moving out of Switzerland?

Does anyone know what paperwork you have to file with the Swiss authorities, specifically Canton Vaud, when moving out of Switzerland?  Thanks K

If you have private health insurance, you need to inform them and send them a "declaration of depart" (and maybe even proof of med insurance in your new country, which is crazy, but what I had to do when I moved over to France.) In tandem with that, I believe you have to send all those documents and a form to the cantonal health authority - "service d'assurance maladie"  http://www.ge.ch/sam/.
