EC trains. There are a few per day, direct to Milan, they take less that 4hrs.
You need to buy them in advance though to reserve seats.

I mean, you need to buy the tickets in advance, not the trains :)

Trains are usually not full so you can purchase ticket at last minute, ticket is available +/- 48 hours. There are 4 direct trains running per day.
Other options are :
- fly to Lugano and take a bus or taxi from Lugano to Milano (1 hour ride) ;
- drive (about 4 hours as well depending on traffic).

well Nicolas it actually depends, if you travel in the weekend at pick times (e.g. the 18h42 train) it is usually full or almost full. So it is always safer to buy in advance.

I did that journey recently driving for the fist time and didn't realise how much the toll roads cost. One cost €29 each way and the Mont Blanc tunnel was €51 return...

Sometimes you get very cheap tickets on the train, if there is a special offer and you buy well in advance. Once I paid smth like 25 CHF for a return ticket
