I'm not an insurance expert, but I'm pretty sure that when you get insurance, it only catches future events, not past events.
In any case, I recommend you contact Zurich Insurance on this, and ask. They give free insurance advice, in English, to glocals memebers.
As per the doctor costs: if it's just a simple thing you're probably looking at CHF 100 or so. From there, the sky is the limit...

You don't necessarnecessarily have them in the same insurance. The first one is easy to get, no insurance can deny it, the other can be refused or the price risen if you have some medical burden.
The other thing you need to know is that you have to chose a so called "franchise". This is the amount you have to pay per year of all your bills. You can choose from a minimum of 300 up to 2000-3000 ChF. A higher franchise will give more affordable rates.....

some corrections :

-The insurance start on the on the 1st of the mounth you sign the insurance proposition ( if you sign on 3rd april or 30th april the basis insurance start on 1st April) and you have 3 mounth to choose an insurance but you re not obliged to start the insurance the day you arrived in Switzerland but if you don t respect this delay, the administration will choose one for you with the minimum franchise and you won t be able to change untill the end of the year.
-If you don t have a job or if work for your own you also have to suscribe accident coverage in your health insurance.
- finally, you can t be refused for the basis insurance, even if you re not healthy cause no medical questionary is asked for the legal insurance.
I work with different insurance ( helsana, groupe mutuel, assura...), I can provide discount on aditional insurance ( complementaire ), so just let me know if you need my help.

I can't say for BUPA in particular, but my insurance is international and certainly does apply for ob/gyn check ups.

Don't forget one thing, you can't change when you want, when you have the lowest franchise you can change for another insurance 1st of july and january, for all the other franchises only 1st of january (with a 3 monthes notice in both case, even if usually you can cancel until the 30th of november because the premiums are getting more expensives).
What you could maybe do, and only by writting them a letter, is to ask them to change it from 01.01.2008, if you didn't had any medical expenses so far. Keep in mind that they don't have to accept it, they only do it sometimes during the first few monthes of the year and as we're mid april, it's prolly a bit late but you can try, not much too lose except 15 minutes.
For who you could deal with... usually for my english-speaking customers I used Intras, it's a good insurance, decent price, and they have enough english speaking people, in case one of them needed to talk with someone in the insurance.
If you need more infos, you have my phone/mail (and maybe I see you wuickly tomorrow actually anyway).
