export dog to usa
i have sucessfully imported my dog from australia 3 years ago.  she is 5 kilo mutt, possibly mix of australian sheep dog and chihuahua. 

now we are going to be moving to the united states in december.  i want to make sure i have all the paperwork in line and find the best airline to travel with.

any and all information would be helpful.
good experiences?
bad experiences?
inside the plane with me?
under the plane? (this is how she flew from oz because it was mandatory)
do it with a company or arrange it directly with the airline?
special crate? where to buy it?
medication?
shots?

 I flew with my cat on Continental and had a great experience, but she flew in the cabin with me.  Perhaps others who have traveled with dogs can recommend better.
Your vet will also be a great source of information.  I know that animals must have a health certificate within ten days of traveling, but perhaps also a preliminary appointment with your vet will help make sure everything is in order.
