Where to make a 3D company logo?

Hello!
I am looking since months for a company who can make a company logo from glass or plastic. Could discuss other materials as well. The logo should be free standing and not too heavy. The size is about 30*15.


Do you know any company doing such an orders? Maybe someone has already a 3D printer? Any online company in your mind?
