Chile Visa -> Health Certificate

Recently received list of requirements for visa to Chile.

One of the items - Health certificate not older than 1 month

First thought - WTF?

Embassy was helpful clarifying this matter and told that "medical certificate requested, must certify that your are healthy and don’t have any serious infectious diseases"

Anyone faced and was able successfully fulfil this requirement? Is there special form for this?
